The Five County AOG Human Services
Directory is published with Community Services Block Grant funding
distributed through the Human Services Department of the Five County
Association of Governments. It is targeted primarily to the under
served population, however, many services apply to all levels of income
and need.
Inclusion in the directory does not constitute endorsement by Five
County Association of Governments or any other agency, person, or
entity represented, or involved in the directory’s production. This
Directory is a collection of human service and health care
information—including agencies, programs, and other resources designed
to promote health and independence for residents of the Five County
region.
The document has an alphabetized index of Services on pages i through
xxiii. The Service Providers are listed
alphabetically on pages 1 through 174. The Directory
is in PDF format and requires a browser plug-in capable of viewing PDF
files.
